3. Breed/Type = (if Mixed) /Size(Small, Medium, Large, Giant)

  • Large 60 to 100 lbs
  • Giant = 100 lbs and up)

4. In Pet Information please include

• • Age/Birthdate and/or "Gotcha Date"
  • Sex & Spay or Neuter
  • Friendly with Children, Dogs, Cats
  • Energy Level
  • Feeding schedule ( 1 or 2 times per day)
  • Potty Breaks (every 2 to every 4 hours)
  • Kennel Trained
  • Veterinarian Information (Name and phone number)
  • Pet Insurance Information (Company and contact number)
  • Health Issue, Medication, Allergies, ect.
  • Likes, dislikes, triggers, training issues, tricks known etc.

5. Vaccination Information = upload their current vaccination records.
